Crescent Community Clinic is NOT an emergency or walk-in clinic. You must call to schedule an appointment, and you will be given a licensed provider for your chronic health condition or mental health illness. You are also asked to bring all medications to each appointment.

We ask you to donate whatever you can afford. Your donation is vital to keep our services available. We are getting ready for our next fall fundraiser.

How to Apply for Services

In order to qualify, you need to be uninsured, low-income or no income, be a resident of Hernando County between the ages of 18 and 64 with a chronic health or mental health condition that the clinic can treat. Please call 352-610-9166 to get pre-qualified and to schedule an appointment. Also, please bring a photo ID, Social Security Card and proof of income and residency with you.

You can fill out this application to save us time getting you into our system.

Applications have a one time $15 processing fee.

OUR PROVIDERS DO NOT PRESCRIBE CONTROLLED SUBSTANCES.

We do not accept patients who take marijuana, either recreationally or medically prescribed.

We do not accept patients who are applying for disability.

Our Health Care Clinics and Health Literacy Programs

Chronic Health Clinic provides screenings for the following chronic health conditions:

  • Arthritis
  • Asthma
  • Diabetes
  • C.O.P.D.
  • Emphysema
  • Epilepsy
  • Heart Disease
  • Hypertension
  • Obesity

On a very limited basis our physicians may refer patients to our partners for their chronic health conditions. No narcotics or pain medication is ever prescribed at the Clinic. Licensed providers diagnose and treat patients.

Dental services are no longer available at the clinic.

The Mental Health Clinic provides psychotherapy, counseling and prescriptions for diagnosed mental illnesses. The Clinic does not provide any alcohol or drug-use services. Licensed psychiatrists, therapists and counselors provide appropriate mental health services. Small group sessions are also available.

The Pharmaceutical Prescription Assistance (PPA) Program is available to help those with a Medicaid Denial Letter to receive up to a year of medication that the manufacturers provide at no cost to the patient.

The ‘Tools to Quit’ Smoking Cessation Program conducted by GulfCoast North Area Health Education provides a two-hour class once a month. Under this program the patient may receive nicotine patches, gum or lozenges and counseling to quit use of tobacco products. All new patients who use tobacco products are required to attend the smoking cessation program.

Diabetes Education and Glucose Testing Kits provide diabetic patients with meal-planning education at no cost. The G.E. glucose testing monitor with lancets and strips are available for a minimal cost.
